In person meeting at Longreach, Maine Maritime Museum, Attendees: Mark Hymbaugh, Tommy, Barbara, Polly, Janet, Jess, Kelly, Karin, Marla, Allison, Deirdra, Jim,

Guest Speaker Mark Hymbaugh with Sagadahoc Country Emergency Management present to collaborate with us:

●      Community Navigation - Case manager role. This time of year getting requests for heat assistance, CMP, food, etc. He likes to get to know programs for screening purposes.

●      Housing is so difficult. Expose people to properties that might fit their situation.

●      Empower people to be self-sustaining. Send people with a plan.

●      Addiction and recovery is a piece of what he did at Lincoln County. His assistance is limited to Sagadahoc County residents.

●      How many counties in Maine have a community navigator? Lincoln and Sagadahoc - Maybe Penbay Y might start?

●      Hospitals have people who do this work. The qualification window is very narrow. Point people to the other groups that have the funding

●      Example: friend hit retirement age, didn’t know what to do next with social security. Helped him make calls, get resources he needed, help with applications, etc.

●      Office hours - will work when people know Mark is there. If we know of an event where Mark could run into the people who’d like assistance, let him know and he’ll show up.

●      3 biggest obstacles in his position - transportation, money, housing.

●      Connection with health officers - he tries to ride along.

●      Community connector - Fed Gov’t someone in Bowdoinham. Tom Mahony / Harpswell

●      Deirdra will share Mark’s email [mhymbaugh@sagadahoccountyme.gov].

●      Mark shared flyers that he uses to make connections with people in need of his services.

●      Nurse Navigators - Mark has met with the head, Kerri Escency. Very similar to what Mark does. Complex Care Case Managers.
